What Is The Model Of Quality Improvement (QI) Project

This quality improvement (QI) project is approached through the model of the improvement plan, do, study, act (PDSA) cycle. This project had four rapid PDSA cycles. Each cycle consisted of four tests of change involving team engagement, patient engagement, and two process measures on patient compliance to medications and the medication’s influence on hypertension (Refer to Table.1). For each team engagement ramp, we have implemented periodic staff meetings, staff education, and team building activities, games, reward points and gift cards for performance improvements as the intervention. For the patient involvement ramp, we have involved the patient in their care through questionnaires on barriers to treatment compliance, organized the patient/family conference to involve both patient and family in their care and also arranged transportation to attend meetings in collaboration with Medicare/Medicaid services. To improve compliance with medications, we have distributed educational materials about the QI project and about the medication(s) given to each patient, arranged 1:1 patient educational sessions with the physician and the pharmacist and collected patient feedback on the changes made to make necessary
