Empowerment theory is a social process that assists individuals in gaining control and fostering power over their own lives and environment. This theory promotes self-efficacy, consciousness, personal responsibility, dual perspective, and a reduction in self-blame. (Langer & Lietz, 2015). Strengths in this theory include the consistency with social work values and ethics; it addresses oppression, discrimination and marginalization on a political and clinical scale, and is in line with the person in environment perspective. Limitations of this theory could be that there are so many skills within the theory, that it would be difficult to assess the actual success rate of using empowerment theory based interventions.

Through discussion of what would benefit the patients, developing a community resource guide to navigate services seemed to be an appropriate support and tool that sufficed. After it was developed, it was added to the paperwork patients receive during discharge. Along with other items, patients are given this resource guide and shown how to use the information, as well as clarify what each part of the guide can be used for.
This intervention is on the macro level; the macro level of practice has the intention of changing the individual’s environment to better fit the individual (Segal, Gerdes, & Steiner, 2015). As I would research one resource, I would think of another, and another, and ended up adding many more sub sections than I originally intended. What comes naturally to me (finding resource and supported to fit my needs), is an unknown area for some of the patients I worked with. I did not understand my limitations until presenting the guide to some of the patients leaving when they revealed they have never even called to make their own appointment before; how would they navigate with this guide? I had a few patients that did not know how to use the internet and a few that would not have access to a phone or the internet. After hearing some of these concerns, I added a resource on the guide that had computer and phone access spots for them to be able to use. In the future, a more detailed guide with addresses and appointment times might be better utilized. Another limitation was that the guide would not work for patients that were not staying in Arizona after treatment; therefore the guide should be given only to patients who will be staying in the state after discharge. My skill set demonstrated within this project was my ability to listen to what the patients wanted help with, and then researching the best resources that were available in the surrounding
