The old way of working in health and social care setting, there are routines had been set up by care provider which is not for the need of their service user, but is made tasks easier for their staff; which means the service they provide is not always needed by the service user and the service is not available for service user when they need it. Take the example of meal times in care homes. The reason I emphasized care home setting is because people will have more freedom in the community than in care home where the care and service is under 24/7. For example, we have a lady who is always very much aware of her weight and she does not want to be too fat. We use MUST tool in our home to monitor resident’s risk of being malnutrition, and her BMI always show her being at risk of malnutrition. By using person centred care, care team had a discussion with the lady to pass on our concern, but the lady expressed her wish of not being too big and she is very happy about her slightly under weight. In respecting this lady’s wish, care team will still offer her normal diet instead of fortified diet which contains high calories and fat menu. On the contrast, if the care team offered her fortified diet and caused the lady to put on weight, she may be distressed and upset about all her clothes getting too small.
