Mrs. Bell demonstrated interest in getting to know her results and to create plans to address the issues found. Therefore, with client-centered approach, the recommendations consider Mrs. Bell’s perspective of her condition as well as her clinical needs and safety.
Cognition
Although client scored in normal range in the screening test (SMMSE), client presented moderate impairment in memory/cognition in other two comprehensive assessments (ILS and CCT). She stated that she tried to do puzzles designed for children and she liked doing those. She said that those ones made for adults were too challenging and she lost her interest.
Recommendations: Client will benefit from receiving assistance to recall mid-term memories in ADLs and IDLs. She said that she would be open to do more cognitive exercises, like puzzles and others.
Safety & Health
Mrs. Bell demonstrated lack of awareness to protect herself when going out at night, also to recognize the potential seriousness of an unhealthy situation. She showed that she relies on other people to problem solve the situations regarding her health and
