The priority learning for my patient is adherence. The patient is a rancher from rural South Dakota. Therefore, he often chooses not to undergo certain treatments and procedures in fear of what will happen to his ranch while he is gone. This can lead to more severe health problems in the future. The patient initially decided not to undergo a CABG this past February when it was recommended, then recently the doctors notified the patient that his condition was worsening and that it was very important to get the surgery completed. For this patient it will be vital to explain the purpose of follow up care and treatment, which includes attending follow up appointments in Rapid City.

Collaboration with team members to support my teaching work with this client, Intra-professional:
• Other nurses to help explain the importance of follow up. Such as having one of the nurses who was in the patients surgery come visit the patient postop and while the patient is alert and oriented and explain their view on follow up care. Interprofessional:
• The physician will encourage the patient on the importance of follow up appointments and what they specifically plan on looking for at each appointment.
• Pharmacy may also come and talk to the patient about follow up care. They may also recommend attending follow up appointments so that the health care team can undergo a medication consultation. Unit resources – teaching material
• Handouts regarding the importance of follow-up appointment after a CABG surgery.
• Handouts regarding the actual surgery and usual process of healing.
