• Update for CORE- if a homeless Veteran arrives with a case worker (or referred by HVRP and VR&E), he don’t need 102 by CORE, refer to DVOP to provide 203 (Proceed same as on Outpost). Case note “Vet is participant of HVRP grant) - because this could create a confusion. Although everyone should know the process, it should be done only when it is known fact that Vet is Chapter 31 participant and is visiting AJC with career counselor, usually by Bolye. If 102 is not provided, please provide case note why (homeless, HVRP, Chapter 31).

    According to the HUD-VASH Resource Guide for Permanent Housing and Clinical Care (2004), “the primary goal of HUD-VASH Case Management is to help eligible Veterans, and their dependents when applicable, obtain and maintain permanent housing.” The case manager begins by completing the Housing Assessment form with the eligible veteran and as this is being completed the case manager is also bearing in mind prospective service opportunities, available resources, and conceivable challenges to housing stability.
