According to CDC (2011), an evaluation plan is a description of how the program is monitored and evaluated as well as how the result of the evaluation can be used to improve the program. There are two kinds of evaluation: formative and summative evaluation. Formative evaluation is conducted during the implementation plan and intends to improve the process of implementation; summative evaluation is typically conducted at the end of the implementation and focuses on the outcome to determine the merit of the implementation plan (Bowen, 2014).
The evaluation of this project are completed within the scopes of formative evaluation and summative evaluation. At the initial phrase of the implementation plan, the printed educational materials are delivered to each apartment. The information on the printed material consists of fall risk self-screening questions, how to prevent a fall, group-based exercise schedules, as well as encouraging the residences to receive a home hazard screening by OT and physical assessment by trained PT. To evaluate an effectiveness of the printed educational material on falls prevention among older adults ages 65 and older in this community is conducted by comparing the collected information at the beginning, 1-month, 2-month, and 3-month period. By analyzing these data, it shows the trends of number of falls or near fall episode, participants in exercise programs, home-screening request, physical assessment request, and home hazard problem during the implementation period of three months.
