Acute Rehab Case Study

How Study to be Conducted This study will focus on protecting patients from fall and fall-related injuries after stroke in an Acute Rehab Unit. At first it would identify the patients who are at great risk of fall currently and after going back to the community. The Morse Fall Risk Assessment (MFA) tool will be used to initially identify fall risk patients in the unit. CDC’s Injury Center has created a tool kit, called STEADI (Stopping Elderly Accidents Deaths and Injuries). It is an algorithm for a fall risk assessment and interventions, designed for the health care providers to assess the risk of falling among people aged 65 years and older. For other risk assessments, patients’ current list of medications will be reviewed, and the problems with syncope will be assessed. The cognitive screening will be done by using Mini-Mental Status Exam (MMSE). The investigator will make sure, if the patients are cognitively capable of signing the consent form, or not. If not, then they will find out, if there is any family member available to sign the consent for them. The investigator will assess their Functional Independence Measurement Score (FIM) on admission to the acute rehab unit and plan the interventions accordingly. Age of the subjects will be 65- 85 years, whose FIM scores expected to be higher to show maximum independency before discharge. While patients stays in the acute rehab unit, the FIM scores will be monitored every shift in order to keep track of their progress or decline in their functional ability.
