Reason for Assessment/Referral: Client agreed to serve as examinee for EDC 503 assessment project.
II. Background Information: J.P. is a thirty-three year old Caucasian male. He is employed as an assistant principal. The examinee lives with his wife of eight years and their two adopted children. He did not disclose any details regarding why they adopted their children. For this assignment, I chose not to ask any questions about possible infertility or other potential issues that contributed to their decision to adopt. He reports that he enjoys a very happy marriage. He has two younger brothers. The examinee reports he lives a basically healthy lifestyle. He is aware that both heart disease and arthritis are common in his family. To his knowledge,

Discussion of Results: The examinee performed well on the mini-mental status exam, missing only a single point. The exam was well received by the examinee. He reported that he had seen this type of test used before with a family member involved in a car accident. The primary insight the examinee gained through the genogram was the consistency of positive relationships between himself and other family members. He mentioned that a recent argument between himself and youngest brother caused him to question how well he maintained family relationships. The genogram helped him to see he actually has very positive relationships with most of his family members. As an ISTJ on the MBTI, the examinee appears to be serious, thorough, realistic, and dependable. ISTJs are sometime called managers of facts and details. This fits clearly with his daily work responsibilities. It also matches many of his results from the 16 PF. The examinee reported that his MBTI results were in line with his understanding of self. We had a great discussion looking through the aspects, characteristics, and descriptions of the different personality types. The 16 PF results supported the examinees MBTI findings. The examinee said these results were fairly accurate, but that completing the test was tiring. He recalled the MBTI during this discussion and stated that, for his time and effort, it was a better tool of self-exploration than the 16 PF. The examinee reported that he believed he may have over thought on the multiple intelligences assessment. He said that he found himself looking for exceptions to most of the statements which may have skewed his results. This is likely true since the results from this test are out of step with the other assessments. He believes that his primary intelligences should rank verbal/linguistic first, followed by interpersonal, followed by musical. The examinee reported that the career assessment was spot on. We had a discussion about how often his day to day work

    KBIT 2 Summary

As a measure of current general intellectual functioning, the KBIT-2 was administered and yielded a Verbal Score = 86±5; a Nonverbal Score = 55±5; and an IQ Composite Score = 66±4. The composite score falls within the very low or borderline educational classification at the 01st percentile rank per national age based norms and needs to be viewed with caution. Due to Hunter’s struggles with focus and difficulty attending to auditory instruction, his nonverbal score is felt to be an under representation of his actual performance. Repeated testing within the near future with this or a similarly constructed psychometric instrument would probably find Hunter scoring higher than an IQ band of 60 - 74. Analysis of KBIT-2 scores found Hunter demonstrating an unbalanced skill set between his verbal skills and his spatial, visually based skills.…

For that reason, I should have used the mini-mental status review with my client to gather information if there was a change in cognitive functioning. What I have learned, maintaining sensitivity and respect of another will enhance the healing relationship. As a result of this practice, I held an intelligent conversation with the client, offered an understanding of how the mental status examination operates, and how information gained can help in decision-making. Also demonstrating flexibility and eagerness to accept mistakes in my own thinking and the ability to prepare recommendations for…

Family Assessment Assignment According to Potter and Perry (2010), a family assessment is necessary to provide sufficient family care and support. A family assessment gives the community health nurse an overall picture of the family’s structure and functions. Family health concerns are acknowledged and family strengths are emphasized by completing a family assessment (Stanhope, Lancaster, Jessup-Falcioni, & Viverais-Dresler, 2011). This paper will examine a family assessment that I conducted with a family and will include a completed genogram (see Appendix A) and ecomap (see Appendix B).…
