The National Health Service (NHS) Commissioning Board (2012) stated the provision and delivery of excellent quality patient care is one of six pivotal values, commonly known as ‘The 6 C’s’, which should be held by all healthcare professionals, however, this can only be achieved with suitable leadership and management. Systematic failures across healthcare have become a prevailing feature, due to the increase focus being placed upon financial measures and performance targets, rather than patient care (Bach et al, 2012; Moss, 2013). Findings from the Mid-Staffordshire Inquiry and Keogh Report (Keogh, 2013, Francis, 2013; see Appendix 2) define the NHS culture as unhealthy and dangerous.
