Clinical case management
Youth Horizons provides a range of services to meet the needs of children, young people and their families/whānau with challenging conduct, anti-social behaviour and/or mental health needs. Youth Horizons use their own strategies which are expressed all over New Zealand with assistances of three major environments. These are in home support which is within the home of a young person. Secondly, community which is supported in a community based surroundings, last but not the least, out of home with the care and support for young people in a family-like home environment.

Enliven services are for people of all backgrounds, cultures and religious beliefs. There are different types of Enliven services available. Within these services support is tailored to the person’s needs, interests and goals. Although the clinical activities specified in this model may also be practiced to varying degrees in the broker model, this model makes explicit the expectation that case managers are clinicians with skills in areas such as psychoeducation and psychotherapy (Lamb 1980). Some example of Enliven services are practical in-home support and personal care services to support people to maintain their independence at home day centres and social clubs for older people respite and day programmes for people living with dementia retirement villages residential care, rest homes and hospitals. Enliven maximises the independence of older people and those with disabilities or injuries. Our home support and community services enable people to live in their own homes, stay healthy and engage socially with others.
