Case Manager explained to client that she is expected to cooperate in developing, carrying out, and completing an ILP together with facility staff to achieve permanent housing. Case Manager explained to client that if she failed to attend meetings without a valid cause, a warning will be issue for non-compliance. Client was also informed that she is to submit a copy of the Birth Certificate, Social Security Card, and identification card.
Exit Strategy Plan: Case Manager asked client if she has an Exit Strategy Plan. Client stated no. Case Manager explained to client that all residents are expected to have an Exit Strategy Plan which consists of searching for employment, savings, and searching for housing.
