Ms. Santiago arrived at Rose McCarthy shelter on Sunday, April 29, 2018. On Tuesday, May 1, 2018 Case Manager Ms. Ijelu met with Ms. Santiago, client to complete her intake and assessment process. CM explained facility rules and regulations to Ms. Santiago. CM completed Ms. Santiago service plan and reviewed ILP goals to client. The following were discussed during the course of the meeting:
REASON FOR HOMELESSNES: Client reported that she came into shelter, due to "her unborn life being threatened by her mother and grandmother” in which she lived for approximately 5 years. In addition, client reports to have had on-going disputes with her grandmother who also living in the small apartment. Client stated the apartment was seriously overcrowded and she slept in the same bed with her father. Client's exit plan at the moment is to secure full-time employment and save towards permanent housing.

Santiago is pregnant; her due date is August 20, 2018. Ms. Santiago noted she is unable to work due to back pain caused by the pregnancy. She is established with prenatal care at Brooklyn Health Center, however client stated she is unsure if she will remain under her mother’s insurance plan. Ms. Santiago must submit documentation of physical exam and TB clearance. Client submitted a copy of WeCare appointment slip at 3pm on May 1. 2018. Client informed CM she was involved in a car accident in January 2018; she is unable to work and also cannot attend physical therapy due to her pregnancy.
Employment: Ms. Santiago is currently unemployed and not seeking employment due to her health status. Client informed CM she has only has retail experience.
Housing: Client is not currently housing ready due to lack of saving and being unemployed. Client was informed about the types of housing vouchers, and LINC programs available that she may qualify for once she obtain full-time
