Summary: Andrew Williams is a 28-year-old male who is a previous customer of this counselor. Andrew’s case was closed-rehab in 2015. Andrew is coming back to MRS as he would like assistance with retaining/maintaining his current job with Kroger. With Andrew’s permission, his mother Arlene joined the meeting. He declined a detailed orientation as he stated he remembers the process. This counselor still provided him with another copy of “The 6 Steps to Vocational Rehabilitation Customer Handbook”.
This counselor assisted with placing Andrew at Kroger in 20015. He was hired in as a courtesy clerk where he stayed for 2 years. In October of 2017, he was promoted to a cashier position. Andrew was doing well and never had any issues. Andrew is a model

This counselor, Andrew, and his mom, Arlene agreed that if Andrew doesn’t get his job back then another meeting would take place to discuss a new job goal, hours, travel, etc. Andrew informed this counselor that he did apply for a camp counselor/play yard attendant with Camp Bow Wow, but decided not to take the position and wait to see how things go with Kroger.

He is coming back to MRS to receive assistance with retaining his current position. This counselor believes that he will require assistance with advocacy, possibly job coaching, and intensive follow-up services to ensure he maintains employment if hired back. If employment is not retained, he will require support to obtain and maintain new employment. Andrew has difficulty with communicating and representing himself, which is why this counselor will work with his union rep to help advocate for him. This will ensure that his employer understands that although he did something wrong, he may not have fully understood what he did and the consequences it would have. If he does receive his job back, job coachi9ng and follow-up services will ensure that proper accommodations are put into place for him to remain
