The person served was engaged into Bridgeway PATH services in March 2011 when she was found homeless, sleeping on a Park Bench in Plainfield, NJ. The PATH team was able to link her to permanent housing in August 2011, and she was transferred to Supportive Housing (SH) on 8/31/2011. The person served was a 57 year old, African American female who resided in her own apartment in Elizabeth, NJ. She had been diagnosed with Bipolar disorder, manic severe with psychotic features, Personality disorder, Congestive heart failure, High blood pressure, and Substance Use Disorder. The individual was diagnosed with Uterine Cancer at the age of 20, which resulted in a hysterectomy. She had a difficult time engaging with all Supportive Housing team members and would only follow up with Team Leader (TL) and some of the counselors on the …show more content…
The person served often had failed visits with the team. She remained in pre-contemplation about mental health services. She expressed wanting to be linked to mental health services, outpatient; however, these appointments resulted in failed visits. On 2/18/15 the team tried to relink her to UCPC, went to transport her and she refused to go. On 2/24/15, the Nurse attempted to link and go to primary care appointment with the individual, and she refused the support stating she would follow up on
