the viscera of the thoracic cavity is made up of what 3 compartments?
1. left lateral pulmonary cavity
2. right lateral pulmonary cavity
3. central mediastinal cavity
what do the 2 lateral pulmonary cavities contain?
1. the lungs
2. visceral pleura: covers surface of the lungs
3. parietal pleura: lines the thoracic cavity
what are the 4 parts of the parietal pleura?
1. costal part
2. mediastinal part
3. diaphragmatic part
4. cervical pleura
what are other names for the costal part of the parietal pleura?
-costovertebral
-costal pleura
what does the endothoracic fascia separate?
the costal part of the parietal pleura from the thoracic wall
what is another name for the mediastinal part of the parietal pleura?
mediastinal pleura
what does the mediastinal part of the parietal pleura cover?
the lateral aspects of the mediastinum
what is the mediastinal part of the parietal pleura continuous with ant, inf, and sup?
1. anteriorly: costal part
2. inferiorly: diaphragmatic part
3. superiorly: cervical pleura
what does the diaphragmatic part of the parietal pleura cover?
the superior surface of the diaphragm except at the pericardium and costal attachments
what attaches the diaphragmatic pleura to the muscular fibers of the diaphragm?
phrenicopleural fascia
where does the cervical pleura extend superiorly?
extends through the superior thoracic aperture into the root of the neck
what does the cervical pleura cover?
covers the apex of the lungs which lie above the first rib
the cervical pleura is a superior continuation of what?
the costal pluera and the mediastinal pleura
what is between the parietal and visceral pluera of the pulmonary cavities?
the intrapleural (pleural) space that is pushed completely together by negative pressure
what does the central mediastinal cavity separate and contain?
separates the 2 pulmonary cavities and contains all thoracic structures besides the lungs
where is one space that the intrapleural space actually exists?
the costodiaphragmatic recess
-here the visceral and parietal pleura are not completely pushed together by negative pressure
why are the parietal and visceral parts not completely attached to each other at the costodiaphragmatic recess?
b/c the lungs do NOT fill the thoracic cavity during complete exhalation
what may accumulate in the costodiaphragmatic recess?
extrapulmonary fluids
-therefore, it is a good place to get pulmonary fluid samples
what is the inferior boundary of the costodiaphragmatic recess?
the superior border of the diaphragm
-forms pleural lined 'gutters' which surround the upward convexity of the dia inside the thoracic walls
what is the superior boundary of the costodiaphragmatic recess?
inferior lung
what is the costomediastinal recess?
where parietal and visceral parts are not completely attached to each other posterior to the sternum
-where the costal and mediastinal pleura meet
what are lines of pleural reflection?
where two pluera meet and form a line
what is the sternal line?
where the costal and mediastinal pleura meet anteriorly
-follows along anterior surface of sternum
what is the costal line?
where the costal and diaphragmatic pleura meet
-exists all around base of diaphragm
what is another name for the costal line?
costal reflection
what is the vertebral line?
where costal and mediastinal pleura meet posteriorly
-follows post aspect of mediastinum
where is the visceral pleura and mediastinal part of parietal pleura continuous with each other?
at the hilum of the lung
-where pulm A, pulm V, and primary bronchus enter and exit the lung
-therefore, airway and vascular supply passes through both parietal and visceral pleura
How many lobes do the lungs have? what divides the lobes of the lungs?
-3 right lobes(divided by oblique and horizontal fissures)
-2 left lobes (divided by oblique fissures)
Which lung is larger, shorter and wider? Why?
the right lung b/c it is pushed up by the diaphragm and diaphragm is pushed up by the liver
Where is the cardia notch? why is it there?
-anterior border of the left lung (primarily indents the anteroinf portion of the sup lobe)
-this is where the midiastinum (heart) resides and pushes in L lung
what is the lingula formed by?What does the lingula do?
-the cardiac notch forms the lingula which is a thing process of the sup lobe of L lung
-it moves in and out of the costomediastinal recess during respiration
(as the lung inflates, it moves down; as lung deflates, it moves back up)
what is the apex of the lung? what is it covered by?
-blunt sup end of lung
-covered by cervical pleura
What part of the lung ascends above the level of rib 1?
apex
What are the 3 surfaces of the lungs?
1.costal surface
2.mediastinal surface
3.diaphragmatic surface
what is the structure of the costal surface? what pleura is it related to? what is the posterior portion of the costal surface called?
-large, smooth, convex
-related to costal pleura
-called vertebral portion of costal surface
What surface of the lung is concave because of the mediastinum? what does this surface include?
-mediastinal surface
-the hilum
which surface is concave? what is different btwn the R and L surfaces and why?
what does this surface form?
-diaphragmatic surface
-concavity is deeper in the right lung b/c rt diaphragmatic dome is higher b/c of liver
-forms the base of the lung
What is the anterior border of the lungs?
costal and mediastinal surfaces meet and overlap heart
What is the inferior border of the lungs?
diaphragmatic surface of lungs
What is the posterior border of the lungs?
costal and mediastinal surfaces meet posteriorly
what is the hilum?
roots of the lungs where lung attaches to rest of body
what is superiormost on L hilus?
pulmonart art
what is anteriormost on hila?
superior pulmonary veins
what is inferiormost on hila?
inferior pulmonary veins
what is posteriormost on hila?
bronchi with bronchial vessels
what is the pleural sleeve? aka?
-where visceral and parietal pleura meet medial to hilum
-mesopneumonium
does intrapleural space have nn's and BV's?
no only intapleural fluid unless there is a problem
