What is the MOA of Digitalis?
Inhibiting the Na/K ATPase causing increased Ca exchange into the cell
What are the effects of Digitalis?
Decreases ESV and EDV-->
Increases CO-->Increases renal perfusion-->reduces sympathetic tone
What is a side effect of Digitalis?
DAD's (increased Ca+)
When are Isoproterenol or Dobutamine used?
These drugs are B agonists. They are used in acute HF.
What is the MOA of Isoproterenol and Dobutamine in HF?
Increases Ca+ uptake, increased rate
What are the names of two phosphodiesterase (PDE) inhibitors used in HF
Amirinone
Milrinone
What is the MOA of the PDE inhibitors?
PDE degrages cAMP. By inhibiting it, you are in effect increasing cAMP levels much like in B agonists.
What is the use of furosamide in CHF?
Reduce edema. Remember that hypokalemia + digoxin = increaded dig effects
What is the use of vasodilators in CHF (nitrates, ACEi, ARB's)
To reduce the afterload strain on the heart
What are 3 commonly used vasodilators in CHF
Minoxidil
Hydralazine
Prazocin
What classes of drugs are recommended as a cocktail for a CHF patient
Diuretics(furosamide), ACEi (nitrates/ARB's second choice), Digoxin, B blockers
In acute HF what is a major difference from chronic HF?
You dont give B-Blockers in acute HF, they have an effect on prevention of remodeling in CHF
