When it comes to person-centered planning, it is nice when a plan becomes a reality and the pathway for actions. This week’s reflection paper will focus on person-centered planning and person-centered care on two specific populations: individuals with disabilities and people struggling with addictions.
It is common knowledge that people with disability labels have been excluded from the mainstream of society for centuries. Due to this lack of access to and involvement in community life and societal privileges, the value of certain groups of people has not been fully realized. Thus, social devaluation of individuals with disabilities and addictions, unfortunately, predetermines their quality of life and how we those consider “normal” interacts

From my perspective, if we learn to value all individuals, such working approaches will become automatic and effective tactics. Before inviting a discussion on the quality of care, we must explore health disparities and why it exists. This discussion will include situational factors behind what most professionals encounter. So, to create a holistic assessment or narrative of patients' situations there are simple actions every hospital and health system can take within their institutions. For example, UPMC could tie the elimination of health care disparities to the organization’s mission. Neighborhoods clinics could utilize community health assessments, forums, and other venues to meet the health needs of the community. In addition, Allegheny County could develop targeted interventions and provide culturally competent, patient-centered care approach for local health entities to model. There are many components within this complex issue: the elimination of devaluation of specific populations. To truly eliminate such notion in health care setting, there must be fundamental and concision changes to our global health delivery system: from providing coverage for all to enabling consistent access to high-quality healthcare to developing national education and communication strategies that represent true equity across all populations and diverse
