The patient is having dementia, hypertension and ESRF stage 4. However, patient reused HD and was under nephro clinic, but planned for palliative management. The patient had been admitted to the hospital due to the same presenting complaint for several times. I choose, this case because of the patient’s interesting background. The patient is an immigrant and was married to local female. The patient is not working and his family cannot afford for any expensive medical treatment. As an immigrant the patient are not subject to any government support, especially in medical treatment. Due to his poor financial status, patient are unable to undergo haemodialysis and buy medication for his diseases. Therefore, the patient and his wife refuse to undergo any treatment and ops for conservative management. According to my supervisor, there is nothing that they can do for the patient except for palliative care as he refuse to get any treatment. I have never seen this situation before, and all this while I have only seen this situation in movies or drama and in previous integrated assignment, but never in real life. When this happened in front of me it really open my eyes. I realised there are still unfortunate people out there that need help with medical assistance, especially with their finances. I really pitied the patient and his family, but as a medical student it is beyond my capability to help the patient. the only thing that I can do for the patient is just words of courage and motivation. Hopefully there are people out there who can do something for the patient and his
