History of present illness: Patient presents to the Emergency Department with complaints of stroke like symptoms. Patient is visibly weak on the left side and slurred speech.
Relevant past medical and surgical history: Patient has a history of hypertension and diabetes.
Admitting diagnosis: Transient Ischemic Attack

Arrhythmias may occur; treatment should be immediately available. Infusion site (and any other potential bleeding sites) should be monitored and patient assessed for hemorrhage during therapy and for 1 hour following therapy. Strict bedrest should be maintained and bleeding precautions should be instituted.

Classification: Therapeutic: antipyretics, nonopioid analgesics; Pharmacologic: salicylates
Therapeutic use: Slows the blood’s clotting action by reducing the clump of platelets, this helps to put a stop to or reduce blood clots.
Major adverse effects: Gastrointestinal bleeding, dyspepsia, epigastric distress and nausea.
Nursing implications: Monitor for the onset of tinnitus, hyperventilation, headache, tinnitus, , agitation, mental confusion, stupor, diarrhea, and sweat. If these symptoms appear, withhold medication and inform physician or other health care professional right away (Vallerand & Sanoski, 2013).
Patient teaching: tell patient that aspirin is given prophylactically and they should take only prescribed dose.
Rationale for medication: Administered when stroke is suspected.
