This study further investigates the effectiveness of MBCT in discontinue or reduction of antidepressant. Similar trials with smaller timeframe (60 weeks) had compared the effectiveness of mindfulness-based cognitive therapy with support to reduce or discontinue antidepressant medication (MBCT-TS) with a therapeutic dose of maintenance antidepressants in preventing the relapse or recurrence of depression. This study extended the population to a larger group with more superior pragmatic trial of MBCT-TS in comparison to m-ADM for patients previously experiencing three or more depression episodes. This is the first definitive randomised controlled trial which compares the MBCT-TS with maintenance antidepressant treatment, which further confirm the effectiveness both clinically and economically. Firstly, recruitment of 424 adult patients with a current diagnosis of major depression in either full or partial remission according to DSM-IV was performed from 95 GP (general practices) and checked for eligibility. The study identified the study parameter of comparing a manual therapy with medication. It also clearly stated its aim to test for efficacy and cost effectiveness of the MBCT-TS in comparison to m-ADM. Outcome measures to compare the two treatments include the primary outcome of time to depressive relapse or recurrence over 2 years and secondary outcome of cost effectiveness, life quality, depression free days etc. were also clarified.
4. Was the study design appropriate for the research question?
RCT was used to perform this study as it provides the highest hierarchy of evidence, and it is the best study design for a quantitative research question. The research question aimed to explore the difference in clinical and cost effectiveness of a therapy with medication. This is a quantitative study with statistical outcome measures and randomised controlled trial was used as the study design in this case. Theoretically, RCT provides the strongest evidence and prevention against bias is maximum. A well conducted RCT should minimise bias to fullest extent
