DSM 5 Diagnosis – Obsessive-Compulsive Disorder 300.0 with good or fair insight
Rationale: Phillip has been presenting with symptoms for years, possibly beginning in his childhood. Phillip meets all criteria for Obsessive-Compulsive Disorder. Phillip has recurrent and persistent thoughts and urges that are experienced at some time during the disturbance, as intrusive and unwanted, and cause marked anxiety. Phillip has thoughts of his girlfriend waking during his morning routine which causes him much anxiety and he also has much anxiety about driving, he has to repeat his route to ensure he hasn’t run someone over. The behaviors or mental acts are aimed at preventing or reducing anxiety or distress, or preventing some dreaded event or situation; however, these behaviors or mental acts are not connect in a realistic way with what they are designed to neutralize or prevent, and are clearly excessive. Phillip gets up an hour early to avoid waking his girlfriend and his drive time is excessive having to repeat his route. The obsessions or compulsions are time-consuming and cause clinically significant distress or impairment in social, occupational, or other important areas of functioning. Phillip has lost his job due to his obsessive compulsive behaviors and his behaviors are causing distress on this personal relationship with his girlfriend. Phillip’s obsessive-compulsive symptoms are not attributable to the physiological effects of a substance is not better explained by symptoms of another mental condition.
