Kirsten, It seems like I write too many of these emails to you. Yesterday Julie, Kathy and I were in psych with 4 pt for most of the time (I got back there at 1). We started with 6 pt, but quickly got down to 4 shortly after Amy and I had switched. 10 had been on seclusion all day and the others were very compliant, although one of them had FYI's. We had just received a new pt in room 8 who was settled and speaking with an advocate, room 9 was asleep, 10 on seclusion and 11 was in the shower but compliant at the time. I was sitting in front of 12 and 13 (who was medical). The pt in 12 had been sleeping most of the day and not in need of a lot of staff support. He did come to the door to ask for something but I couldn't hear him. I got …show more content…
A quick glance at the camera I observed staff having difficulty getting the pt in a good position on the bed due to his size. Gina and April (security), MD Gillette, Nancy RT, RN Jaime H, Abby, Jamie C and I (there could have been more) were trying to get the moved up in bed. I went to head of bed with Gillette to help pull pt up. An OPA was inserted but the BVM hadn't been taken out yet so I grabbed it and gave it to Nancy then I hooked it up to O2. Nancy said she needed help holding the mask to get a seal. I took over bagging, Gillette held and Nancy went to get her ventilator set up. Kathy came to the doorway of 13 and said "you need to clear, I need your help over here." I said that Amy just walked by and could help. I looked over and noticed we didn't have any suction in the room. I asked Eva to go get suction. Thankfully Gillette is awesome at intubating and got it without suction. I continued to bag while Nancy set up the ventilator. Nancy placed the pt on the ventilator, but it was not working. I continued to bag while Gillette was ensuring tube placement. There was a prolonged period that the ventilator was not working so the pt needed to be bagged. Pt was moved to 16 and assisted staff in rolling the pt for x-ray. I then cleared and returned to

It is vital that the team help educate the patient with medication, how to use instruments that they are exposed to, and therapeutic skills that they can do once they are discharged. It is essential that teams observe the patient and let them demonstrate what they’ve been taught. Breath sounds are normally clear or scattered fine crackles at bases, which clear with deep breathing. The presence of coarse crackles during late respiration indicates an airway obstruction, teams should help educate each other on the patient's condition and what to expect to care for the patient more effectively. The semi fowlers position, with the head of the bed elevated 45 degrees, allows the diaphragm to expand.…
