Chronic Disease And Its Effects On Health And Welfare Essays
In Australian context, chronic disease makes upto 70% of the disease burden and it is estimated to increase upto 80% by 2020. Previously, NSW health system was designed to focus more on acute centric care model, however with time there was need for increased illness prevention, health survellience and support for chronic illness management (Rochester 2015). As a result, government has proposed some reform over few decades with its focus on care and management of individual with chronicity. New reforms in health policy emphasizes on primary health care that works on enhancing and enabling the role of practiceners focusing more on prevention and management (NSW Health 2014).
However, there is still a need of more emphasis on the role of practice nurses in order to provide interventions needed to develop subject to local needs (NSW Health 2014).
