Cayuga Medical Center Internship Report

My internship placement was at Cayuga Medical Center, in their Clinical Resource Management Department. The mission statement for the facility is to deliver the highest quality health care in partnership with the community, one person at a time. Their core values are: clinical excellence, customer service, people, financial intergrity, and community. Which from working there I could tell that they take their mission statement very seriously, because while going through training that was one of the main focuses of the sessions. As their committment to contribute knowledge about aging and the older population could be self explainatory if you just visited the hospital and observed how many older adults individuals are treated there. The number of older patients in the hospital compared to the middle or younger aged patients is so much higher, with some type of representation of the community ofcourse but nonetheless, CMC is committed to the all populations but especially the older population. One of the main communications formally that the hospital has into place is called, “rounds.” Rounds are when the Hospitalist and Regristered Nurses, and Nurse Case Managers and the Director of Clinical Resource Management gather in a room every weekday at nine o’clock AM to discuss patient status and their discharge plans. The informal communication that they have with one another, I believe is just having eachother personal contact numbers, therefore if one of them have a question or an issue they can be on-call. Where I think could be beneficial for the patients, is that the Hospitalist and other professionals communicate more often about the more difficult patients, and that they come to an agreement about the plan of action for patient so that they won’t confuse the
