Case Study: Follow-Up After 30-Day

CM received a call from Ms. Marshall (caregiver) in regard to a follow-up after 30-Day (CFT) meeting with Alexia (youth) at Daytop. Caregiver and CM discussed the Strength and Needs Assessment and the crisis plan; needs and strategies were discussed and family vision was created. CM explained to caregiver some struggles that youth are currently facing at the program. CM informed caregiver that youth reports she feel that her mother does not care about her. Caregiver expressed that she is not able to contact youth while she is on Phrase 6; CM encouraged caregiver to be more proactive in youth’s treatment. Also, CM encouraged caregiver to reach out to Jaimie (therapist at Daytop) and Kelly (Case manager) to discuss treatment.
