At my agency, the program I currently work with is a part of Governor Cuomo’s three-point plan to end Acquired Immune Deficiency Syndrome (AIDS) as an epidemic in New York State. The plan includes identifying individuals with HIV who remain undiagnosed and link them to health care. Second, link and retains individuals diagnosed with HIV in health care to maximize virus suppression, so they remain healthy and prevent further transmission. Third, facilitates access to Pre-Exposure Prophylaxis (PrEP) for high-risk individuals to keep them HIV negative. Said strategies are set forth to decrease new HIV infections to the point where by 2020, the number of persons living with HIV in New York State will be reduced for the first time.
Upon enrolling into the Linkage and Navigation program, a requirement is for all clients to have a Comprehensive Assessment to get a better understanding of the client’s system. This information is entered into our electronic health records system. Nevertheless, the assessment is based on a psycho-social model; it consists of twenty-six pages which are to be completed within forty-five days over the course of three interviews. Two interviews are conducted in the office, and the other is done in the client’s home. This protocol was implemented to observe the client in the formal and informal setting.
