Examples of many of these issues were seen in the Asiana flight 214 crash. There were two examples in this crash of inadequate supervision. The first being that the pilot monitoring (PM) of this flight was on his first flight as an instructor pilot. He was instructing a trainee captain on operating experience (OE). During the PM’s instructors training he was never given the opportunity to practice this. If he had it might have allowed the PM to be more aware of the dynamic and ready for a situation like this one (NTSB, 2014). The second example was the fact that the pilot flying (PF) was unfamiliar with the autopilot and auto-throttle systems of the Boeing 777. This crash has led the FAA to reconsider just how automation is looked at in the industry. The NTSB also made many recommendations to Asiana and Boeing to avoid this situation in the future. Many of these being developments in the training department for this aircraft. All other airlines should learn from this crash and look into their training of automation to make sure their pilots have a extensive knowledge of automation. This would help to avoid any crashes similar to this one in the
