What is the significance of IgM ab in the diagnosis of congenital virus infection?
It means current infection. Exposure has been 1-2 weeks.
Is the presence of IgG antibody in an infant significant in diagnosis of a congenital infection?
It means that the infection has been going on longer or it could be maternal ab. You need IgM from neonatal blood to prove that it is a congenital disease

At 2 weeks this is the predominant type of ab.
What is the primary route of infection of an infant with rubella? hepatitis B? Herpes type 2, Cytomegalovirus, HIV?
Rubella- resp. droplets
CMV- type of herpes virus- transmit by infected saliva, breast milk, sexually and through infected blood. Transmission to the fetus is after primary or recurrent infection.
HSV 1/2 are transmit perinatally during passage through birth canal.
Parvovirus- resp. route. Very infectious.
HIV is transmit mainly during birth. It is only rarely passed in breast milk.
Hep B is mainly transmit during birth.
What are the reasons to be concerned about hepatitis B infection in the newborn? How is it treated?
Affected kids will become carriers and have congenital disease.
They can get cirrhosis or hepatocellular carcinoma.
Treated with HBIG- hep B immunoglobulin.
When during pregnancy is an infection with rubella of most concern? Why?
0-12 weeks 100%
it crosses the placenta in monocytes.
After 16 weeks there is normal development with slight risk of abnormalities
What is the causative agent of fifth disease?
Erythema infectiosum caused by Parvovirus
What test would you order if your patient was exposed to a child with slap cheek disease during her first trimester?
Slap cheek is parvovirus
