What does OSE stand for?

Occupational safety and ergonomics

True or false: safety is multi-disciplinary

True

In a facility, who owns safety, ergonomics, or Human Factors?

Nobody

True or false: products and systems must be designed for reasonably foreseeable use and misuse.

True

**True or false: accidents have single causes and can usually be classified strictly into one category

False. They are multi-factorial and cannot be classified strictly into one category

What is an accident?

An undesirable event that interferes with normal work process and carries the potential for physical injury, property damage, or product loss.

An accident may be attributed to a human factor (both intentional and unintentional), a situational factors (operations, tools, equipment and/or materials), or an environmental factor (both physical and psychological).

What is a hazard?

A condition that has the potential to cause injury (It doesn’t have to cause injury), damage to equipment or facilities, loss of material or property, or a decrease in the capability to perform a prescribed function.

What is danger?

The probability of loss. It is dependent on the relative exposure to a hazard.

i.e. transformer in a vault is a high hazard but people cannot easily access it.

What is damage?

The severity of injury or magnitude of loss that results from an uncontrolled hazard.

i.e. walking on a balance beam 10 feet above the ground versus 100 feet above the ground may present similar exposure, levels of damage.

What is risk?

A function of the probability of loss (danger) and the magnitude of potential loss (damage).

Probability of loss * magnitude of potential loss

What is safety?

The control of hazards to an acceptable level

Safety is the function of the industry or environment you are in.

i.e. soldier in war. Person on ski slope.

True or false: safety is the absence of something bad happening.

False

What is the hierarchy of safety controls?

1) Engineering controls

2) Administrative controls

3) Personal protective equipment

Engineering Controls are at the top of the hierarchy.

________ is an intangible but important aspect of any overall safety effort.

Safety culture

True or false: personal protective equipment can increase other risk factors.

True. Factors such as heat stress and required force. PPE should be carefully chosen.
