One of the fastest growing chronic conditions in Australia is diabetes. About 85% represents Type 2 diabetes mellitus (T2DM) and is still rising in prevalence (Diabetes Australia, 2015). In managing this condition, the focus is to control the blood glucose levels to the normal range as possible in order to prevent further complications. Though, controlling this problem is immensely demanding for individuals with Down syndrome and an intellectual disability (ID). This essay will analyse the health issues and determine the possible effect on Joe’s health and wellbeing. At the same time, it will also assist him to manage his T2DM while concurrently adapting to life with ID. Moreover, it will also examine how Joe’s activity and participation are possibly affected by a chronic illness and an intellectual disability through utilising the International Classification of Functioning (ICF). Finally, to achieve Joe’s optimal health and wellbeing nurses must identify a strategy and intervention for managing Type 2 diabetes that is tailored to Joe’s condition.
