For example; gaining knowledge of, age, gender, ethnic groups, educational level, income, prominent illness/disease, life expectancy, health care access, and if the population have health insurance or are without coverage all shape and determine community need and compliance to health promotion. The statistics about community aid the CHN in development and delivery of specific needs and its execution all the services supplied.

CHN’s must gain thorough understanding of the community they serve by research of the area to develop population need and determine resources available to provide healthcare services. Gathering information about this Phoenix zip code was eye opening for me. I have lived in Maricopa County for all but 3.5 years of my life and I did not understand the poverty levels and struggles that surrounds me. Zip code 85007 is approximately 25 miles away from my home, but seems like another world. Assessment of the community has provided me with the tools to develop or coordinate care for this population and perhaps even make a difference. Working within the community you must tailor education to meet the needs and comprehension of the population and you must overcome barriers to communications. Barriers to healthcare for a CHN include; cultural, language, education level, and socioeconomic differences and these should be considered by the CHN. Nurses and CHN’s alike must establish a rapport with their patient population for maximum healthcare services (Oudshoorn, Ward-Griffin, Forchuk, Berman, and Poland, 2012).
