What is the pathogenesis of acne?
multifactorial
includes hyperkeratosis with blockage of the pilosebaceous canal, increased growth of Proprionibacterium acnes, overproduction of sebum, and inflammation.
Androgen stimulation increases sebaceous glands production causing increased cell turnover and cohesiveness of keratinocytes at the pilosebaceous canal. This causes the keratinous material in the follicle to become more sticky and block the canal
The blocked pilosebaceous gland produces a small cystic swelling causing a raised area referred to as a microcomedome

whitehead: the mouth of the pilosebaceous canal remains closed

gram (-) anaerobic diphtheroid bacteria
secretes a low molecular weight chemotactic factor and lipase enzyme that breaks down triglycerides of sebum into free fatty acids - this breakdown causes irritiation of the follicular walls which leads to the papules on skin
What cells are involve in cyst formation?
neutrophils release hydrolase which disrupts the integritiy of the follicular wall which damages the underlying dermis this forms a pustule --> If this inflammation continues it will eventually form a nodule and subsequently a cyst
