Case 1: Ford Pinto

Ford Pinto fuel tank ruptured due to a collision - which found Ford to be negligent



Gas tank design was flawed and not in line with accepted engineering standards even though it met federal safety standards



*the engineers were well aware of the dangers of the design beforehand



they figured that the cost of implementing the changes was too high - as compared to just letting the design flaw stay and risking many lives which was monetarily cheaper

Case 2: Therac-25

Radiotherapy machine that was software controlled, with few hardware safety mechanisms



Had multiple design errors and safety mechanism failures



Lack of resources to develop software - only 1 engineer



Lack of testing - they ******* cb use the same damn control software from last gen on the older machine



Lack of urgency - initial denial of the situation and slow response to reports of the problem
