VA Sierra Nevada Health Care System Case Study

Improved Essays
The VA Sierra Nevada Health Care System (VASNHCS) has a current compliance rate of hand hygiene on the floor of less than 50% with bi-annual visual monitoring. This monitoring is carried out by infection control staff members on a 5-day bi-annual basis and may be inaccurate due to basis and visibility limitations. Reporting to the joint commission requires that the VASNHS show improvement with the goal being a compliance rate of 90% or better. The VASHCS has failed to show improvement of 10% each reporting period.
To bring the hospital into compliance by showing evidenced-based improvement and reducing hospital acquired infections requires a more regulated and protocoled hand hygiene routine on al medical floors. Implementing the Hill-Rom

Compare and contrast UCLA and Intermountain Medical Center’s outcomes. Why do you think there are such significant differences in outcomes and expenditures? • Both UCLA and intermountain medical centers, I believe, have a goal to improve the health care system and the outcomes for patients. The UCLA medical system was struggling to match the outcomes that were being seen at the intermountain Medical Center despite the expenditures. I found it interesting the difference in what procedures are being performed and at what rate.…

Although medical and nursing staff employ the use of standard and additional precautions the single most important thing that staff can do whilst working is perform effective hand washing to reduce the risks of cross contamination. Everyone has some form of resident flora on their body, this helps the host to stay healthy but when this resident flora is transferred by contact to another person this may cause the person to become ill. In addition to this resident flora a person picks up infectious particles through contact with other people or environmental surroundings (Koutoukidis et al, 2013, p 361). The most effective way an enrolled nurse or other health care workers can decrease the risk of transmission of these infectious organisms is…

Once Congress and the Secretary (with competent accountants) mull over the needs of the VA health system, the money woes should be figured out satisfactory. Only through accessing a problem with both the parties supplying the money and those spending the money can a mismanagement of funds be settled. This also has the added benefit of justification of expenses and should prove that funding should not be reduce for the sake of those who risked their lives to fight for and defend this country. Inadequate scheduling is not just a problem for the VA, but any of the government related healthcare systems. Trying to do more with less is fine for a corporate paper pusher with no real consequences, but when you are risking the lives of countless numbers…

Hand Hygiene is considered “the most important measure for preventing the spread of pathogens in health- care settings” because it reduces the spread of pathogenic organisms to patients and health- care workers. Studies in the mid 1800s showed that hospital acquired nosocomial infections and diseases were spread by the hands of healthcare workers. In the 1980s, the first national guidelines for hand washing were published, and many other guidelines followed. The US public health service released a training video in 1961 that demonstrated recommended hand washing techniques for healthcare workers. The video recommended washing hands with soap and water for one to two minutes before and after coming in contact with a patient.…

During my clinical rotation at Palmetto Richland on the Acute Care for the Elderly unit there were several patients on contact precautions due primarily to Methicillin-resistant Staphylococcus aureus (MRSA) or high counts of gram-positive Escherichia coli bacterium (E. coli) in the urine and blood. A good example of best practice that I witnessed was the proper display and implementation of contact precautions around patients with multidrug-resistant organisms and bacteria (MDROs). If a patient is placed on contact precautions on this ACE unit floor, a large blue hanging bag, called an isolation precaution bag, is hung outside their room containing gloves and gowns. The nurse is to put on the glove and gown before entering the room and making contact with the patient.…

As the amount of veterans that return home from war increases, the VA Hospitals will begin to get extremely backed up. 6 month waiting times will turn to to 7 months, then 8, and so on. Around the nation, there is a total of 152 VA Hospitals, which help care for roughly 8.3 million veterans a year (“Veterans Health”). That is roughly 54.6 thousand people per hospital just being treated per year. That means that more than 1,000 veterans have to be treated every week in any given hospital.…

According to the Centers for Disease Control and Prevention [CDC] (2014), the primary mission of the institute is to protect the American people from safety, health, and security threats, both regionally and globally, through scientific research and information to the public. I feel that this is an experience that every student in healthcare should go through. One of the most important observations that I made concerns the various procedures of preventing infections in the operating room. The prevention majorly focuses on the patient, especially when considered the patient has open sounds during the operating process. At times, the practitioners tend to forget the importance of cleaning their hands with soap and water or with alcohol-based cleaners when interacting with the environment (Scott, Earl, Leaper, Massey, Mewburn, Williams, 1999). Notably, the assumption that the practitioner does not need to clean hands simply because they have gloves is misplaced, and it is often overlooked (Weber, Anderson, &Rutala, 2013; Karki& Cheng, 201)).…

Introduction According to Virginia Department of Health (Hand Hygiene, 2013) , hand hygiene is defined as a method of removing microorganisms from the hands so the germs cannot be transmitted to anyone else. The two most common types of hand hygiene are hand washing antimicrobial soap and alcohol-based hand rubs. Proper hand hygiene helps reduce the spread of infections especially to those individuals who come in contact with people on a daily basis. Hand hygiene is very imperative to those working in healthcare settings no matter if you come in direct contact with a client or just by sitting behind a desk.…

Lastly it was found that infection control measures should be implemented within three days after onset of symptoms of infection and more (). Nevertheless, the infection control measures are effective. Taking precaution and following a strict protocol resulted in the positive outcome. However, this intervention could be ineffective if individuals do not adhere to the standards of the protocols within specific time frames and the use of specific amounts of cleaners. In all, I have learned that by implementing protocols and strategies this could greatly reduce the risk of the spread of germs and infections.…

Hand hygiene is described by many health care workers as the single most important tool in preventing the spread of health care-associated infections between patients. According to WHO, there are few definitive data on the patient-care activities that are most likely to transmit bacteria to health care worker (HCW) hands, but there have been several studies that identified many possibilities. Although bacteria have been found on HCW hands after activities such as wound care, intravascular catheter care, respiratory tract care and handling patient secretions as expected, bacteria also have been found on HCW hands after so-called “clean” contact, such as taking a patient’s pulse, temperature or blood pressure.…

All the studies comes done to the fact that proper hand washing technique can prevent the spread of those diseases. “Every year, lives are lost because of the spread of infections in hospitals. Health care workers can take steps to prevent the spread of infectious diseases. These steps are part of infection control” (Medline Plus).…

Hospital acquired infections are particularly challenging because many of these infections have developed antibiotic resistance (Pollack, 2010; Rosenberg, 2015). However, most hospital acquired infections are preventable through a variety of techniques, including the use of hand washing, equipment sterilization, use of gloves, sanitizing surfaces, among other things (Mehta et al., 2014; Monistrol, Calbo, Riera, Nicolas, Font, Freixas, & Garau, 2012; U.S Department of Health and Human Services, 2015). The key here is consistency, and a hospital culture committed to reducing and even eliminating hospital acquired…
