Suicidal Case Study Summary

Mr. Lamond is a 40 year old male who presented to the ED with suicidal ideation with a plan to lay down in the street and let a truck run over him due to being homeless, the local homeless shelter not letting him stay last night, and being tired of drug use. He also reports consuming 8 40oz beers today and smoking marijuana. Per documentation on 1/27/17 Mr. Lamond reported being kicked out of local homeless shelter due to drug use. At the time of the assessment Mr. Lamonds endorse suicidal ideation, however does not specify a specific plan. When confronted about earlier comments about suicidal plan, he states, "They didn't tell you", and never confirms a plan. Mr. Lamonds reports recently being release from jail on 3/10/17 due to some recent charges. Mr. Lamonds states Jeff, the owner of the local shelter, told him last night he didn't have any bed and could not sleep on the floor. He reports since his release on the 10th he has been staying in a abandon house. Mr. Lamond states, "I didn't want to go …show more content…
Lamonds most recent UDS was positive only for Benzodiazepine. His BAC was a .21. Mr. Lamonds reports this when he woke up around 8 or 9 he started to consume a few Natural ice and through the day smoked 2 "blunts" or $20 worth of marijuana. He states, "Yeah I smoked that good stuff." He expressed recent withdrawal symptoms as headaches, tremors, and sweats. Mr. Lamonds reports he was offered substance abuse treatment apon release from FHMR, however refused it. He states, "I want it now." This clinician was supportive and explain the process he would possible have to go through to receive detox treatment.

Dur to increase in depressive symptoms, suicidal ideation with a plan, patient does meet criteria for IVC and inpatient hospitalization. TACT consulted with Dr. Osborne and Courtney Schenck, LCSWA, it was recommended to further monitor over the course of the night an conduct a status check later on in the morning.
