Client “MDC” is a 44 year old who has been living with her boyfriend for the past two years in the streets, shelter, and motel. They have no children together; however the client has three children from a previous marriage. Client presented with multiple concerns related domestic violence abuse, depression, employment, and concerns over alcohol/drug abuse from her current ex-boyfriend.
Client is diagnosis with major depression (DSM-5 Diagnosis: Major Depression-F32.9) due to the fact her ex-boyfriend sexual, emotional, mental and physical abuse by pulling her hair, unwilling and forceful sexual abuse while staying a motel for couple days. Abuser took the client psychiatry medications and use force and gave the client cocaine all day/night while during sexual intercourse. The client actually escape the motel room and ran to the clerk to inform him to call the police that night; while the abuser was getting arrested for domestic violence and assault he was stating to the client “ I would kill you when I get out”. The abuser received 20 days in jail; client

She did maintain her focus on the session and answered each question during the counseling session. Client would like to focus on concerns related to her domestic violence. Client stated she would like to have individual therapy sessions with a female therapist and attend a domestic violence group when she is comfortable to speak or listen in a group setting. Client is worry about not have a normal lifestyle again due to her looking over her shoulder all the times; due to her ex-boyfriend was released from jail. As for the legal/ethical part the client signed all informed consent forms. She was notified of my status as an intern and the name of the supervisor. She verbally informed of the limits of confidentiality and signed the acknowledgment of HIPPA guidelines
