DOI: 8/21/2015. Patient is a 31-year-old male janitor who sustained injuries to his wrist/hand, neck and back when he slipped and fell on loose bathroom tiles. Per OMNI, he was initially diagnosed with cervical strain, lumbar sprain/strain and right hand contusion.
Based on the progress report dated 01/07/16, the patient complains of lumbosacral pain, rated as 9-10/10 with radiation into the bilateral buttocks. Arching of the back and twisting aggravate the pain. Forward flexion does not cause as much pain.
He underwent the care of chiropractic manipulation therapy/physical therapy which helped to a limited extent. EMG/NCV studies revealed bilateral motor tibial nerves lesion on 09/30/15 (no official report). He has tried Advil and
