DOI: 8/16/2010. Patient is a year old female home attendant who sustained multiple injuries when she fell off a chair.
Per the IME report dated 10/07/14 by Dr. Rubinshteyn, no ongoing PT or orthopedic treatment would be reasonable or necessary. The patient should follow a home exercise program. Permanency rating is Class 3 Severity B in regards to the lumbar spine.
Based on the latest medical report dated 06/09/16, the patient presents complaining of progressive worsening chronic refractory pain in the right shoulder, right hand, right knee and lower back, left greater than the right side for the last two weeks and over the counter pain medications offered temporary/incomplete relief./ She has remaining morning stiffness,
