Challenges of New Graduate Nurses: Transition to Practice

Job retention rates amongst newly graduate nurses tend to fluctuate widely. These fluctuations have many possible and combined explanations including orientation quality, level of confidence, residency programs, pay rates, mentoring programs, management support, and countless views of job satisfaction. My concept will focus on transition to practice success of new graduate nurses in relationship to their organization’s orientation process, job satisfaction, and the effects of job retention rates.

According to Twibell, Pierre, Johnson, Barton, Davis, Kidd, and Rook (2012) in their article Tripping over the Welcome Mat: Why New Nurses Don’t Stay and What the Evidence Says We Can Do about It, new nurses are not satisfied with their jobs primarily because of heavy workloads and questionable patient safety (Twibell et al. 2012). New nurses also sometimes have tough relationships with seasoned peers and management. Ten percent of graduated nurses feel confident starting their practice, and ten percent of nursing executives feel that new nurses are prepared to begin safely caring for patients (Twibell et al. 2012).
Twibell et al. (2012) explains that a residency program is the key to retaining new nurses. Retention rates for organizations with a residency program range from 86-96%. “A residency program involving 679 NGNs at 12 sites across the United States showed reductions in stress for NGNs, improved clinical and communication skills, and a 1-year termination rate of 12%” (Twibell et al. 2012).
