Thoracenthesis

Is the process by which fluid or air is aspirated from the pleural space.

Purposes of thoracenthesis

1. To reduce fluid or air from the pleural cavity.


2. To obtain specimen( pleural fluids for analysis.


3. To relief respiratory distress.


4. To instill medication into the pleural space.


5. To allow for pleural biopsy.


Indications for thoracenthesis

1. Empyema


2. Pleural effusion


3. Congestive heart failure


4. Pneumonia


5. Cancer


Contraindications of thoracenthesis

Thoracenthesis is contraindicated in patients with bleeding disorders.

Nursing responsibilities before the procedure

1. Assemble all requirements at the patients bedside or in the treatment room2 Explain the procedure and purpose to the patient. Infom him that he may feelSome discomfort and a sensation of pressure during needle insertion.3 Administer prescribed sedative, if patient is anxious/cough depressant if patientis coughing4. Monitor patient's vital signs; i.e. pulse, respiration and blood pressure5. Have the patient's x-ray available to determine the site for aspiration6. Note any drug allergies especially to the local anesthetic agent7 Obtain an informed consent8. Expose the patient entire chest/back as appropriate9. Shave the aspiration site as ordered10. provide privacy and give emotional support11. positionpatient and make sure he is firmly supported


Nurses responsibilities during the procedure

1.Check and record vital signs regularly.2. Support the patient verbally throughout the procedure and keep him informed ofeach step.3. Assess patient for signs of anxiety and provide reassurance as necessary.4. Observe patient for signs of distress, pallor, vertigo, faintness, weak and rapidpulse, and decreased blood pressure.


Nurses responsibilities of thoracenthesis after procedure

1. After the needle is withdrawn pressure is applied over the puncture site, andSmal airtight sterile dressing is fixed in place2. Advice the patient that he/she wil be on bed rest and a chest xray will be obtained after thoracentesis3 Monitor patient's vital signs and the dressing for drainage every 15minutes for1hour and continue to assess the patient's vital signs and respiratory status as indicated by his condition.4. Encourage deep breathing and coughing exercise5. Discard disposable equipment, clean non-disposable equipment and return them for sterilization.6. Label the specimen properly and send it to the laboratory for investigation7. Monitor the colour and amount of the drainage Record the total amount of fluidwithdrawn from the procedure8. Document and report any abnormalities9. Administer any prescribed antibiotics10. Dress wound aseptically to prevent wound infections.11. monitor patient for at intervals for increasing respiratory rate, asymmetry inrespiratory movement vertigo, tightness in the chest, uncontrollable cough, bloodfrothy mucus, a rapid pulse and signs of hypoxaemia.
