Skin Turgor

Elasticity of the skin



Abnormal= tenting when gently pinched upward



Checked on forehead or sternum of an adult or the abdomen of a child

Skin Texture

Character or surface of skin and how thick it feels

Edema

Local or generalized condition in which body tissue contains an excessive amount of fluid in the interstitial spaces



Palpate over bony prominences: press for 5 seconds and measure any present indentation

Crepitation

A crackling sound heard in certain diseases within skin or joints



Can be heard in the ends of broken bones

Diaphoresis

Profuse sweating, especially induced sweating



To assess, feel skin for moisture

Erythema

Redness of the skin



An observation that could indicate skin irritation, injury, or inflammation

Pruritis

An itching presenting as a tingling or faintly burning skin sensation that prompts a person to rub scratch

Hirsutism

Excessive growth of hair or the presence of hair in unusual places

Absence or loss of hair

Usually not visible in skin until severe; best assessed in eyes

Cyanosis

A blue/grey discoloration of the skin or mucous membrane caused by deoxygenated or reduced hemoglobin



Most visible in lips and fingernails

Pallor

Lack of color or paleness



Visible in the face, buccal mucosa, and conjuctiva
