There are a substantial number of people now living with healthcare conditions, Health care budgets are under extreme pressure. Working person centred alongside health care professional colleagues and people whom we care for supports people to develop more knowledge understanding and skills that may help in supporting people to make informed choices i.e. promoting person cent care. : "By patient values we mean the unique preferences, concerns and expectations each patient brings to a clinical encounter and which must be integrated into clinical decisions if they are to serve the patient" (Sackett et al 2000) In 2013 the Francis report was released after numerous failures. The Francis report made 290 recommendations to prevent these failures in future (Francis, 2013). This is a central component of person centeredness. Without active participation there is no person centred practise, because individuals are not being fully included in their own care. It’s easy to assume that active participation enhances the well-being and quality of life of individuals. But I like to evaluate my practise to see if this is actually the case and to ensure that the delivery of care is consistent. - In order to implement systems and process that promote active participation it is necessary to put the individual at the centre of everything I do. By encouraging active participation the individual becomes an active member of the team, not a passive recipient of care. Good communication skills between everyone involved in their care and support is the key to implementing person centred active participation and also in determining how to deliver a service, which represents the individual’s
