I would ask Mr. Smith's wife for her husband's full name and date of birth to be sure I checked the right chart. I would then ask her to hold so I could check the chart. I would look to see if Mr.Smith had place on any restrictions on release of his information. If there was no note in the chart restricting the release of information, I would let tell Mr. Smith's wife. However, to prevent any confusion, I would suggest my office use a pin system to prevent mistakes in interperting infomration in the chart. I would incorporate the R in the PRICE model to this situation, by being very respectful with Mrs. Smith advising her what information she was entitled to. If the chart restricted the information, I would apologize and advise Mrs. Smith
