An electronic health record (EHR) is a real time digital version of a patient’s paper chart that make information available instantly and securely to authorized users. EHR contain a patient’s medical history, diagnoses, medications, treatment plans, immunization dates, allergies, radiology images, and laboratory and test results. Allow access to evidence-based tools that providers can use to make decisions about a patient’s care. It 's important to understand the difference between EHR and electronic medical record (EMR) that is a digital version of a paper chart that contains all of a patient’s medical history from one practice. An EMR is mostly used by providers for diagnosis and treatment.

EHRs are built to share information with other health care providers and organizations such as laboratories, specialists, medical imaging facilities, pharmacies, hospitals, nursing homes, and even across states; and is an effective tool to incorporate the patient to the plan of care because allows them to access their charts from any computer after creating a secure password.
For years the government is been straggling to protect patients information, the Health Insurance Portability and Accountability Act (HIPAA) was enacted by the United States Congress in 1996.
