What is the difference between primary and secondary TIN?

Primary is when the disease starts interstitially
Secondary is where the disease starts in the vascular or nephron itself. |

What is a key clinical finding of TIN?

Sterile pyuria and WBC casts
also will see a lack of proteinuria and hypoalbuminemia concentrating defect causing polyuria and nocturia RTA long term will see anemia |

How does TIN present?
|
abrupt onset renal dysfunction - polyuria, nocturia
|

What is the classic triad of TIN?
|
Rash, fever, eosinophilia
but only seen 30% of the time |

What is the treatment of TIN?
|
Remove offending drug
get a renal biopsy Corticosteroid with either prednisone or solumedrol |

When does TIN classically appear?
|
10-20 days after onset of drug therapy
|

What will you find in the infiltrate in TIN?
|
T-lymphocytes, monocytes, plasma cell, and eosinophils
|

What does TIN look like on ultrasound?
|
normal to enlarged kidneys with increased cortical echogenicity
