Stage I

May include changes in skin temp, tissue consistency (firm,boggy,mushy) or sensation (pain/itching)

Stage II

Loss of epidermis. Appears as shallow crater or blister with red pink wound bed

Stage III

Full thickness tissue loss. Subcutaneous fat may be visible.

May be relieved with wet to dry dressing

Stage IV

Extensive damage to underlying structures. Exposed bones, tendons or muscles.
