Congenital club foot is a severe inward rotation of the ankle, foot, and lower leg fixed in an abnormal condition in 1 or a combination of 4 deformities:
1. Plantar flextion - toes pointed down
2. Dorsiflexion - toes pointed up
3. Vargus - foot turned in
4. Valgus - foot turned out
When does tx begin for club foot?
Immediately and continues for 8-12wks, w/ foot placed in cast in overcorrected position
Casts are changed q1-2wks b/c of rapid growth
Patient teaching for a Pavlik harness?
1. Worn 24h/day for 3-6mos
2. Infant must be sponge bathed
3. T-shirt and knee socks should be worn under brace to protect skin
4. Diaper should be placed under straps and changed w/o removing harness
5. Ensure child has adequate stimulation at appropriate eye level
What is the tx for osteomyelitis?
Long term ABX therapy
1. IV ABX for 3-6wks
2. Oral ABX for 2wks following IV ABX therapy
