Distinguishing features

Depressed mood or loss of pleasure during same 2 week period representing change in functioning most of the day, nearly every day and enough of 7 other symptoms to have 5+ most of the day, nearly every day

7% overall in US


3 times higher in 18-29 age group than 60+ age group

Course and Onset

Peak onset: 20s


Recovery starts in 3 months (40%) to 1 yr (80%)


Less recurrence with shorter duration and longer remission


Suicide is 17 times more likely

Diagnostic Criteria

A. 5 or more of following symptoms present during same 2 week period and represent change from previous functioning; at least 1 symptom is either depressed mood or loss of interest or pleasure


1. Depressed mood most or nearly every day


2. Loss of interest or pleasure in all or almost all activities most or nearly every day


3. Significant weight loss when not dieting or weight gain or decrease or increase in appetite nearly every day


4. Insomnia or hyper Solomon’s nearly every day


5. Psychomotor agitation or slowness nearly every day


6. Fatigue or loss of energy nearly every day


7. Feelings of worthlessness or excessive or inappropriate guilt nearly every day


8. Diminished ability to think or concentrate or indecisiveness nearly every day


9. Recurrent thoughts of death, recurrent suicidal ideation without a specific plan or suicide attempt or plan for committing suicide


B. Cause clinically significant distress


C. Not attributable to substance or medical condition


A-C represent MDD


D. At least one MDD episode is not better explained by schizoaffective disorder


E. Never been a manic or hypomanic episode
