Dopamine Hydrochloride (_____)
Intropin
Dopamine Class
Sympathomimetic (both Alpha and Beta properties)
Dopamine Actions
- At low and medium doses, selectively dilates blood vessels supplying the brain, kidneys, heart and gastrointestinal track.
- At medium to high doses, increases cardiac output by increasing contractility and stroke volume, thereby increasing blood pressure. - At high dose, causes vasoconstriction and increased heart rate. |
Dopamine Onset
< 5 min
Dopamine Duration
approximately 10 min after drip is stopped
Dopamine Indications
- Discomfort/Pain of ? cardiac origin with associated shock
- Shock: Normovolemic (anaphylactic, neurogenic, septic) - Bradycardia (after max atropine or after initiation of TCP) |
Dopamine Dosage/Route
ADULT ONLY:
- 10-40 mcg/kg/min in 250 ml NS or D5W IV drip: titrate to BP of > or equal to 90 systolic BHO |
Dopamine Contraindications
None
Dopamine Side Effects
- At high infusion rates, hypertension and extreme vasoconstriction may occur
- At low infusion rates, hypotension may occur - Tachyarrhythmias / palpation - Increased myocardial O2 demand |
Dopamine Concentration
400 mg in 250 ml NS = 1600 mcg/ml
Dopamine Special Information #1
Tissue necrosis may occur with infiltration. If this occurs, discontinue the IV, circle infiltrated area and inform receiving hospital.
Dopamine Special Information #2
Titrate to BP > or equal to 90 but not to exceed 120 systolic
Dopamine Special Information #3
To get a quick estimate of dose:
- To convert mcg/kg/min to actual drops per minute, multiply desired mcg x kg x .039 - To get the 10 mcg/kg/min drip rate, take the weight in pounds, drop the last number and subtract 10% and then multiply x 2 |
Dopamine Special Information #4
Always run as piggyback drip, never run as primary IV.
Dopamine Special Information #5
Assessing the effectiveness of Dopamine administration:
- Check the blood pressure or heart rate |
